How do I do to make pressing alt+enter not entering windowed mode from fullscreen?

Anyway, from the wiki (SCREEN command):

Some games don't work very well in windowed mode - or vice-versa.  For example, on a high-res screen a 320x200 game doesn't look very good in windowed mode.  Although I personally like being able to switch back and forth, it does seem a bit unprofessional to allow that.  Probably better to have such switching implemented on some Options menu.
